Best Portable ECG Monitor

abstract background

Even with heart disease being the leading cause of death in the United States, cardiac diagnostic services are still not readily or evenly available across the nation. For example, a 2025 study found that hospitals in urban areas were more than twice as likely to offer cardiology services as hospitals in rural regions. Already, rural areas experience worse health outcomes and even higher rates of premature death due to severe shortages in healthcare providers, hospital closures, and lengthy travel distances to care centers.

One way to address this imbalance in cardiac care is through portable devices like ECG monitors that clinical teams can easily transport to remote or rural areas without extensive setup or equipment. Below, we’ve put together an evaluation of four of the best portable ECG monitors currently available. We compared them across lead count, software/platform offerings, wireless mobility and set up, analysis or review pathways, and their clinical deployment fit, to identify which options best serve buyers.

GlobalMed TotalECG®, for telehealth-integrated cardiac diagnostics

TotalECG® was designed for organizations that needed the ability to collect cardiac data outside of the traditional boundaries of a clinical setting. With just one click, the ECG device connects to any Windows® computer and requires no external power or connection to an ECG cart. Its main advantage is that it combines hardware, analysis tools, and telehealth connectivity all in one place, thanks to its wireless 12-lead acquisition with CardioVu software, Glasgow analysis, and eNcounter® integration.

This supports the full process of collecting, reviewing, and sharing cardiac data with remote providers, meaning that instead of having a cardiologist at every care site, staff can immediately share or stream ECG data to a cardiologist at a more central location. TotalECG® is the strongest fit for buyers looking for the best portable cardiac monitor for telehealth rather than a device that operates separately from the rest of their care platform.

AliveCor Kardia 12L, for simplified point-of-care ECG acquisition

The AliveCor Kardia 12L is best suited for point-of-care settings like primary or urgent care clinics. This portable 12-lead ECG collects cardiac data via a five-electrode, single-cable design that’s simple to set up. It also runs on battery power, so it doesn’t need a fixed ECG cart or outlet. The ECG recording can be tracked and analyzed using the KardiaStation app, which features AI-assisted analysis to help providers identify 39 types of cardiac findings.

Kardia 12L is well-suited for organizations looking for portable 12-lead testing with guided capture and built-in analysis. However, the Kardia software is separate from a healthcare provider’s telehealth platform and EHR system. Providers can share recordings from the Kardia app, but organizations that need those recordings in another system may need to manually transfer the data or pursue a separate integration option.

QT Medical PCA 500, for cloud-connected 12-lead ECG testing

The QT Medical PCA 500 is a small 12-lead ECG recorder for environments like clinics and remote testing. Its prepositioned electrode system makes placement easier, and it uses Bluetooth to connect the recorder to QT Medical’s ECG apps on compatible mobile devices or computers. Those apps then send the ECG recordings to ECGcloud®, where clinicians can store, review, compare, and create ECG reports. ECGcloud® gives organizations a dedicated place to manage ECG recordings across locations, but it also includes tools for EMR integrations and an API.

The PCA 500 is a good fit for health organizations that value portability and cloud-based record management. The tradeoff is that ECGcloud® adds another software tool to the clinical workflow. Organizations using an existing EHR or telehealth platform may need to dedicate extra time and resources to connecting these separate systems, or staff may need to export and transfer ECG reports themselves.

SmartHeart Pro, for ECG review services

The SmartHeart Pro is a portable 12-lead ECG that connects to mobile devices through Bluetooth. Its chest-belt design helps clinicians capture a full ECG in about 30 seconds, which can be accessed in the SmartHeart app and SmartHeart Portal. The service includes unlimited 24/7 preliminary reads by SHL Telemedicine’s clinical staff and on-demand input from licensed cardiologists.

The main benefit of SmartHeart Pro is the access it gives buyers to professional ECG review without having a cardiologist at every care site. However, using SmartHeart Pro depends on SmartHeart’s app, portal, and review service. Healthcare organizations may find it difficult to transfer reports to their existing systems, or providers may struggle to toggle between separate apps and workflows during appointments.

Why consumer ECG devices are different

Consumer ECG devices are useful for quick rhythm checks, but they do not serve the same purpose as clinical 12-lead systems. The main differences are the number and placement of leads, the amount of cardiac information captured, and the purpose of the recording. For example:

  • Apple Watch: Produces a heart tracing similar to what you would get from a single-lead Lead I ECG. The watch’s ECG app can give the wearer basic heart rate and rhythm information for personal use, rather than a comprehensive clinician-led cardiac assessment.
  • KardiaMobile 6L: Records six limb leads through the Kardia app. It provides more rhythm information than a single-lead device, but it’s missing the data you would get from the six chest leads included in a 12-lead ECG. Because this provides a less comprehensive view of the individual’s heart function, it’s best for personal rhythm checks and sharing recordings with a licensed provider.
