Jacque Sokolov

Over the past 15 years, Dr. Sokolov has worked with top-tier academic health centers, hospital systems, health plans, physician organizations, emerging technology companies and private equity firms in virtually every major health care sector and market in the country. Dr. Sokolov serves as Chairman and Chief Executive Officer of SSB Solutions, Inc. (SSB) and Healthcare Community Development Group, LLC (HCDG). He also serves as a member of the Board of Directors of MedCath Corporation (NASDAQ:MDTH), the National Health Foundation (NHF), Phoenix Childrens Hospital and My Health Direct, Inc.
Dr. Sokolov received his BA in Medicine, Magna Cum Laude, Phi Beta Kappa from the University of Southern California and his M.D. from the USC School of Medicine. He completed his internal medicine residency at the Mayo Graduate School of Medicine and his fellowship in cardiovascular diseases from the University of Texas-Southwestern Medical School. He previously held and currently holds academic appointments and advisory board responsibilities in the Schools of Medicine, Business and Public Health at Harvard, the Massachusetts Institute of Technology, the University of Pennsylvania, the University of California, Los Angeles, and the University of Southern California.
Joel E. Barthelemy

As Founder and CEO of GlobalMed, Mr. Barthelemy has a vision of quality, timely care for all through connected health. He has spent 14 of his 30 years in the technology industry focused on the development of connected health systems. Under Mr. Barthelemys leadership, GlobalMed has improved the delivery of healthcare in over 55 countries, has designed the system that have helped to deliver over 2.1 million consults annually to US veterans in 2014 (the largest deployment of Telemedicine in the world), and has been recognized as an outstanding innovator, community advocate, and a high-growth tech company leader.
Active in his community and industry, Mr. Barthelemy currently sits on the boards of Sun Health Services, Arizona Bio Industry, is an Ambassador to the Veterans Medical Leadership Council and is a board advisor for SEE4Vets. He has served as a board member of the American Telemedicine Association (ATA), where he chaired the ATAs Industry Council, and was a board member of the Visual Communications Industry Group, a high-tech industry think tank. Barthelemy and GlobalMed give back to the community through generous commitments to programs including: Toys for Tots, the Jennifer Lopez Family Foundation, Blanket the City, the Wounded Warrior Project, the Diaper Bank of Central Arizona, the American Heart Association, the United Blood Services Blood Drive, Phoenix Theatre, Celebrity Fight Night/Mohamed Ali Parkinsons Foundation, and the Andrea Bocelli Foundation.
Mr. Barthelemy attended St. Cloud State University, a certificate from Thunderbird School of International Management.
Conrad Prusak

Since 1982 Conrad Prusak has been an advisor to executive management focused on increasing the effectiveness of leadership, maximizing organizational performance and building top-tier teams. He is currently an international leadership coach serving the needs of boards of directors and management teams on two continents as well as a Master Chair for Vistage International, a leading private peer advisory group firm.
Mr. Prusak received his AB from the University of Notre Dame and his MBA in Management from the Graduate School of Business, Stanford University. He also earned certificates at the University of Zurich and the Graduate School of Public Administration in St. Gallen, Switzerland.
